Information for you and your advisors

Creating a legacy of giving beyond your lifetime requires wisdom, vision and a plan tailored to your unique circumstances. You can choose from a variety of options ranging from naming your favorite a MultiCare program or foundation program or foundation as a beneficiary of your will to establishing a trust.

Regardless of the size of your gift or the type you choose, the greatest benefit of a legacy gift to a MultiCare program or foundation is knowing that you will help provide the best health care for all, throughout every stage of life, for generations to come.

A gift through your will or trust (a bequest) or a through a beneficiary designation to a MultiCare program or foundation:

  • Is easy to arrange.
  • Will not alter your current lifestyle in any way.
  • Can be easily modified throughout your lifetime to address your changing needs.

There are a variety of ways to make a legacy gift. Sample bequest language, and the foundations legal names and federal tax ID numbers are offered below for your advisors' consideration in preparing your will, trust, beneficiary designation or other gift to a MultiCare program or foundation. Please note: As a result of the corporate merger of MultiCare Health Foundation, Good Samaritan Foundation [Tax ID #: 91-2004312], and MultiCare South King Health Foundation [Tax ID #46-5636491], of which MultiCare was the sole Member of each, MultiCare Health Foundation [Tax ID #: 91-1514257] was the surviving entity and successor-in-interest pursuant to the Articles and Plan of Merger filed with the Washington Secretary of State effective September 1, 2019. MultiCare Health Foundation became "MultiCare Foundations" ("Foundation"), as amended by the Articles of Amendment filed with the Washington Secretary of State. The aforementioned foundations, to include MultiCare Health Foundation, are each registered DBAs operating underneath the Foundation.
